If you prefer to transfer your care to another healthcare provider, please call 513-977-8881 and leave a message requesting your medical records and include your full name with spelling, date of birth, and phone number. Your message will be returned within 48 hours. You do not need to make an appointment, or see a physician in order to transfer your records.
